Training Facility Rules

The following rules are acknowledged by both player and parent/guardian completing this form on behalf of player if under the age of 18. By acknowledging the rules you acknowledge that if rules are not followed Precision Hockey Training reserves the right to deny access to the training facility or may request the individual who violates any rules to vacate the facility. Repeated violations will be met with a verbal and/or written notice that the individual is not permitted to enter the facility and it will be considered trespassing if they come into the facility without expressed permission from one of the staff.

1. Outside foods and/or drinks are not permitted inside of the training facility. Water bottles/sports drinks that can be capped are allowed.
2. Players, parents, and coaches will respect the training facility, all PHT staff, equipment, other athletes, parents, coaches, and anyone else inside of the training facility.
3. Players, hockey coaches, parents, and children are not permitted to use the equipment inside the facility unless specifically training with a coach during a training session.
4. Parents of players are only permitted to watch during private and group sessions (not team sessions). During team sessions, coaches are welcome to watch and encourage their team, but are not permitted to instruct players.
5. Players are not allowed to wear their own headphones or wear clothing that contains obscene images, gestures, or words. Players must also dress appropriately for training (gloves, stick, skates, running/training shoes, shorts/athletic pants, and shirt)
6. Only skates are permitted on the synthetic ice. Players must be aware of other players on the ice while shooting, skating, and handling pucks. Under no circumstances are players permitted to shoot towards the bench area or flip pucks up in the air.
7. Athletes will not interfere with another athlete performing an exercise/drill. Equipment must be returned at the end of a session. Athletes who are unable/unwilling to train may be asked to leave.

Assumption of Risk

I acknowledge and agree that: 1) The risk of injury from the activities in the training facility is significant and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist. 2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation in any activities with Precision Hockey Training. 3) I willingly agree to comply with the stated customary terms and conditions for my participation. If, however I observe an unusual significant hazard during my presence or participation, I will remove myself from the participation and advise a Precision Hockey Training coach or staff member of the potential hazard. 4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless Precision Hockey Training LLC, its directors, officers, officials, agents, volunteers, and/or employees, other participants, sponsoring agencies, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releases"), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law. I have read this Release of Liability and Assumption of Risk Agreement, fully understand its terms, understand that I have given up substantial rights by agreeing to it, and agree freely and voluntarily without any inducement.

IF PLAYER IS UNDER THE AGE OF 18 AT TIME OF REGISTRATION - This is to certify that you, as parent/guardian with legal responsibility for the participant, do consent and agree to his/her release as provided above of all the Releasees, and for yourself, your heirs, assigns, and next of kin, you release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to your minor child's involvement or participation in these programs as provided above, even if arising from their negligence, to the fullest extent permitted by law.
